Patient Information



We will assist you with all areas of insurance payment. It is necessary for you to bring your insurance card and coverage booklet to the first visit for us to give you advice on what is covered.  If your spouse also has insurance please bring that information as well.  It is very important that we have the correct spelling of your name(s), as well as the correct address and birthday(s).

Payment Plans are available.


Please arrive at least one half hour before your first appointment to permit us to get acquainted with you and to fill out the necessary forms.   A complete medical form is necessary to enable us to serve you. It permits us to become familiar with your total health, which medications you are taking, what medication we may safely prescribe to you and how much treatment we can offer at one visit.

Please bring a complete list of your medications, including doses and the number of times a day that you are taking them. You should carry this information with you at all times.

Please tell us if anything has changed at any future appointments.

If you have had x-rays recently, it would be extremely helpful if you could arrange to have them with you, or sent to our office before your visit.

In order to determine the best treatment options for you, we will:

  1. Listen to what your concerns are: pain, swelling, or broken, crooked, discoloured teeth, lost fillings, sore gums…
  2. Check your mouth inside and out.
  3. Look at your x-rays; they show the condition of your teeth, especially under the gums.
